Below is a list of patient forms and informational documents, which may be downloaded and printed from your computer. To download:

  1. Right-click (PC users) or Ctrl-click (Mac users) the link to the document.
  2. Select Save Target As or Save Link As and save the file to the desired location on your computer.

    Print and fill out this form if your child is 12 years or under.
  • Prenatal Intake Form (139.31 kb)
    Fill this out if you are pregnant and coming to Dr. Thyr for all of your prenatal care, or if you are coming for conjunctive care.

    All new patients should print this form. Read and sign it and bring it to your visit.

    All new patients age 13 and older should print and fill out this form for their initial visit.

    Print, fill out, and fax back to Dr. Thyr if you need any records sent to another provider.
